Season 3: I can't remember clearly, but I'd bet ten dollars to a doughnut it was either Davis or TK.
Season 1-2: I don't know, but I'm certain he wasn't in the cast. (I don't know any of the voice actors)

Actually, you can fight him an infinite number of times, but other than that, yes.

I only counted the ones whom the DigiDestined Spirit Evolved into, and Flamemon had seemed more of a hallucination to me. Anyway, who's Strabimon? I don't remember that one.

By your use of the term "Chosen", I assume you mean Adventure/02 only.
